RMA Request Form


To obtain an RMA number, please fill out this form as completely as possible. An RMA number will be sent to your email address shortly after the form is submitted, following authentication.

Note: HR Telecom, Pacific Crest's regional service center in Europe, does not require an RMA number, whereas all other service centers do require an RMA number for all radio repairs.

Note: Pacific Crest in the US is the ONLY location for RMA returns for all GNSS equipment.

Note: Pacific Crest is no longer accepting RFM radios for service repair.

Beginning April 1st  2011, Shipping charges will no longer be included as part of the flat rate costs on
any “Non Warranty Repairs”. Those charges will be in addition to the repair cost.

Reminders:

  1. Please expect a delay in response, from several minutes up to several hours, after the form is submitted. This is due to Spam and Junk Mail blocker settings in individual e-mail accounts.
  2. Please check your 'junk e-mail/bulk' box, as the e-mail from Pacific Crest may appear there.
  3. After the first e-mail from Pacific Crest is received, please click on the provided link, which will open a new webpage with your RMA number. This number will then be e-mailed to you as well.
  4. Please write the RMA number on the shipping label, the outside packaging, and on a label placed directly on the radio.
  5. Please include a copy of the RMA confirmation with the shipment.
