
To obtain an RMA number, please fill out this form as completely as possible. An RMA number will be sent to your email address shortly after the form is submitted, following authentication.
Note: HR Telecom, Pacific Crest's regional service center in Europe, does not require an RMA number, whereas the North America and Asia service centers do require an RMA number for all radio repairs.
Reminders:
- Please expect a delay in response, from several minutes up to several hours, after the form is submitted. This is due to Spam and Junk Mail blocker settings in individual e-mail accounts.
- Please check your 'junk e-mail/bulk' box, as the e-mail from Pacific Crest may appear there.
- After the first e-mail from Pacific Crest is received, please click on the provided link, which will open a new webpage with your RMA number. This number will then be e-mailed to you as well.
- Please write the RMA number on the shipping label, the outside packaging, and on a label placed directly on the radio.
- Please include a copy of the RMA confirmation with the shipment.
